1. Customer cannot obtain cover (age, residency, bankruptcy, no income).
2. Wrong/invalid contact details (unreachable after 3 attempts/48 hrs).
3. Duplicate lead (within 90 days).
4. No valid consent (audit trail fails).

Customer Changed Mind – The prospect decided not to go ahead after the call, which is outside our control.
Competitor Chosen – The lead went with another provider after speaking to you.
